Florida state law requires clients under the age of 18 to have notarized parental consent before obtaining a body piercing. We've provided the form below- you can print it and take it to any notary, who will watch the parent sign the form and use their notary seal to make the form official. The parent will need to come to the studio with the underage client and provide their identification before the piercing.
